To register for the Family Conference programs and events, please download and complete the registration form & return with your payment to the no later than February 15, 2004.

You must register for your hotel room separately from the conference activities. To do so, contact the Doubletree's reservations system at the telephone numbers listed below. A special room block rate of $125 per night has been reserved for Family Conference attendees. To ensure you get this special rate give the reservationist our Group/Convention Code "ISM" when you make your room reservation. Conference attendees are responsible for their own rooms and all incidentals associated with their room.

Scholarships
Due to the important nature of this first-ever conference on Glycoprotein & Related Storage Diseases, ISMRD wishes to assist any family with a financial need so that they are able to attend. Funds are limited, so, in order that we may consider your request for a scholarship, please provide the information on the Financial Scholarship Application. The maximum amount to be awarded to one family will be $1,000 US. Please return this application filled out in full to the address indicated on the form, postmarked no later than February 1, 2004.

Supervised childcare will be available from 8 am to 6 pm the following dates: April 2d & April 3rd and from 8 am to 12 pm on April 4th .
